Staff and Physician COVID-19 Immunizations Registration
The COVID-19 vaccine is only available to all employees and staff - 2nd dose only.
We are pausing registration for any 1st doses of the COVID-19 vaccine. Clinics will remain open to complete 2nd doses only. We will reopen for 1st doses pending vaccine availability. Individuals in tier 1 who have not received the first dose: please contact Employee Health at ICH: x16223, QVH: x23433 or FPH: x33484 or email Meredith Invencion, Director of Employee Health.
During the registration process, there is an option to add family members. At this time, Emanate Health is not offering any vaccine to family members. Please leave it blank.
Emanate Health ID Badge & Vaccination Card Required
You must present your Emanate Health ID badge and vaccination card when checking in for the vaccine.
Please note that if you are more than 15 minutes late for your scheduled appointment, your dose will surrendered to an employee on the waitlist.
Walk-ins are not allowed.
If you sign up for the 'Waiting List', please wait to be contacted pending vaccine availability. If there are doses remaining, you will be contacted by phone to receive the dose. Due to vaccine instability and time constraints, if those contacted on the waitlist are not available to take the call, EH will move down the wait list.

Covid-19 vaccinations may pause while we await our next allocation from the state. Please consider scheduling an appointment at one of the available LACDPH sites below to avoid any delays. New clinic dates will be released once our next shipment of vaccines arrives.
