We believe that every stage of life's journey is meaningful. That's why Emanate Health Hospice (formerly known as Citrus Valley Hospice) works tirelessly to give our patients superior medical and emotional care as they near the end of their lives.
When to think about hospice
- You or your loved one have a life-threatening illness.
- You or a loved one make frequent visits to the ER.
- Day-to-day living is very uncomfortable for you or a loved one.
- Treatment is no longer effective.
- You or your loved one have a diminished quality of life.
We offer inpatient hospice care and hospice care at home, nursing home or board and care. Our services have a four-star rating from the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS). We are also certified by the Joint Commission.
- The Emanate Health inpatient hospice unit is a 10-bed facility offering short-term crisis management, symptom control and family respite care. The inpatient unit also has plenty of space for loved ones to gather and be nearby.
- For patients seeking more independence, hospice care at home may be an option.
How we care for patients
We have a team of experts who specialize in end-of-life care working for our hospice patients and families. This team includes physicians, nurses, home health aides, a social worker, pharmacist, dietitian, hospice volunteers and a chaplain.
Our skilled staff works together to plan the services the patient's care requires. Here are some examples of the services we offer:
- Dietary counseling
- Nursing care (intermittent visits)
- Physical and occupational therapy
- Volunteer services such as reading, shopping and friendly social visits
- Hospice bath aides
- Spiritual counselors who can provide spiritual support
- Medical social workers
- Placement specialist (helping patients find a place to call home)
We also keep a hospice nurse and doctor on call 24 hours a day, seven days a week to support our patients day or night.
Who pays for hospice?
- Medicare or Medi-Cal and most insurance companies
- Medicare and Medi-Cal covers hospice services fully.
- Medicare covers most comfort medications as recommended by the medical director.
- Hospice covers most durable medical equipment (DME).
- Medicare and Medi-Cal covers hospice services fully.
How we care for loved ones
When it comes to hospice care, it's not just the patient who needs support. Our hospice support groups and classes are available to help family work through their emotions in a loving environment.
Classes and groups for loved ones include:
- Adult group grief sessions
- Emotional maintenance
- Grief outreach
- Road to survival
- Widows and widowers
