We know that some health conditions take longer to heal than others. And we also know that not everyone wants to spend all of their recovery time in the hospital. That's why we offer home care services.
With Emanate Health Home Care, we come to you. You'll receive specialized, physician-directed care for your illnesses or injuries within the sanctuary of your own home.
The best part? It's less expensive than an extended hospital stay, but it gives you access to our expert hospital medical staff while allowing you to be closer to your loved ones. Emanate Health home care is part of the Emanate Health network which includes Emanate Health Inter-Community Hospital, Emanate Health Queen of the Valley Hospital and Emanate Health Foothill Presbyterian Hospital. This relationship allows us to offer most of the same services the hospitals provide with the same high standards.
Emanate Health Home Care has been in the community for more than 30 years and is accredited by the Joint Commission. We're also a Medicare-certified service with a four-star rating from the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey.
Patients who benefit most from home health care
Home care isn't for everybody, but it's an excellent choice for anyone who will need extra help with recovery after they leave the hospital. We mostly prescribe home care to people who are 18 years and older and who:
- Are recently discharged from the hospital and not yet fully recovered
- Have a chronic illness requiring frequent check-ins (such as stroke, diabetes, kidney disease, or heart or lung disease)
- Need a pain management program
- Require intravenous antibiotic therapy
- Need wound care education
- Are on a new medication (such as insulin, an inhaler, or injections)
- Have limited mobility, stroke, fracture or are an elderly patient needing therapy to get back to active daily living
Care from our specially trained staff: We offer a team approach
Our highly trained professionals provide a wide range of services 24 hours a day, 7 days a week.
Our skilled nurses can instruct you on new medications and wound care, check your blood pressure and lung sounds, and report any problems to your doctor. This is covered by Medicare and most insurances.
Our physical therapists can help regain lost balance, strength or mobility. Their specialized approach to rehabilitation can help improve the speed of your recovery.
Our occupational therapists can teach you how to safely perform your daily tasks, such as dressing, bathing, cooking or gardening.
Our speech pathologists can provide therapy and training for your communication and cognition needs, and swallowing after an illness or condition that has affected these abilities.
Our medical social workers can help you find medical services that pay for a person to come into your home to help you, along with community resources.
Our home health aides or bath nurses (covered by most insurances) can help you or your family when you get home from the hospital when a nurse or therapist visits.
Our volunteers (when a nurse or physical therapist is involved) can provide companionship, reading and light meal preparation and can take out your trash can on weekly pickup days.
How do you get home health care?
You'll need a physician's order to be considered for home health care. Once we've received your physician's order, one of our liaisons will meet with you one-on-one to see if you're eligible.
